Here you will:
- Create your account
- Identify each member of your household to be included
- Purchase either "individual" or "entire household" dental plan
- Select your method of payment

You will notice that there is a one-time registration fee of $20.00 for either dental plan.
Once your application is submitted, you will receive a confirmation e-mail to verify that you have actually purchased the dental plan. At the top of this e-mail will be an account number starting with the letter “M” – this will be the same number that appears on your dental plan membership card. In the event, you need dental services prior to receiving your dental plan membership packet, you can give your account number to a dental plan network dentist and receive the discounted dental fee. |
| In approximately 7-10 days, you will receive your dental plan membership packet which will contain – membership cards for each member you enrolled, a dental plan network provider directory for your area, and other literature about the plan. |
