Enrolling in the dental plan is quite simple, but there is a certain amount of information you will need to have on hand to complete the dental plan application:
- You will need basic information on yourself – Name; address; e-mail address; etc.
- You will asked to create a “password” for your account, so be thinking of something you can easily remember.
- If you are enrolling in a “family” or “entire household” dental plan, you will need the names and dates of birth for each person to be included in the dental plan.
- You will be asked to select the type dental plan you want (the abbreviation DVPC stands for – dental, vision, prescription drugs, and chiropractic.) If you just want the dental plan, you will select either the $11.95 individual plan or the $19.95 entire household plan.
- At this point, you will see that there is a one-time registration fee of $20.00 for either dental plan.
- Next, you will select your method of payment – either credit card or electronic funds transfer from a bank account. (Remember, if you are paying by electronic check, you will need the name of the bank, the bank routing number, and your account number.
- Once your application is submitted, you will receive a confirmation e-mail to verify that you have actually purchased the dental plan. At the top of this e-mail will be an account number starting with the letter “M” – this will be the same number that appears on your dental plan membership card. In the event, you need dental services prior to receiving your dental plan membership packet, you can give your account number to a dental plan network dentist and receive the discounted dental fee.
- In approximately 7-10 days, you will receive your dental plan membership packet which will contain – membership cards for each member you enrolled, a dental plan network provider directory for your area, and other literature about the plan.
