Members can save 25% to 65% on all restorative and cosmetic work (fillings, crowns, braces, etc.) and up to 80% on preventative work (teeth cleaning, X-rays, etc.) performed by a general dentist. Specialist fees are discounted 25%. Since the dental plan is NOT insurance or a health care organization, all ongoing dental problems are accepted, except orthodontic treatment in progress. There are no deductibles, no claim forms to fill out and no limits on visits to your dentist. How do I pay my Dental Plan Membership Fee? Monthly or quarterly payments are made only by electronic bank draft or by credit card. Invoicing is done only for annual memberships paid one year in advance. Individual membership is $11.95 per month and family membership is $19.95 per month. Family membership includes all residents in the household, including children, parents, relatives and significant others. How much more do the pharmacy, vision and chiropractic plans cost? The prescription drug, vision and chiropractic plans are absolutely FREE with the dental plans or health benefit plans membership.
Who are the Dental, Pharmacy, Vision and Chiropractic Care providers? We have one of the largest networks of dental plan providers in the nation, and we are continually adding to this base. These dentists are well established, with successful dental practices. Our prescription card may be used at our participating pharmacies nationwide, as well as through mail order. The vision care benefits are available nationwide at more than 12,000 providers, and our chiropractic benefits may be used with more than 7,500 credentialed chiropractors. How do I use the Dental Plans? Within 10 to 14 working days of receipt of your dental plan application, you will receive a Member Information Guide, Membership Identification Cards and a Dental, Pharmacy, Vision and Chiropractic Care Directory. Present your Identification Card to any network dental provider and you will be eligible to get the discounted fees. You pay the provider at the time of service. How does the 30-day guarantee work? If you are not satisfied with the dental plan within the first 30 days of becoming a member, you may cancel your membership in writing. Your membership fee will be refunded. Please be aware that the one-time administrative fee and money paid for healthcare services and products are not refundable. After 30 days dental plan membership fees are not refundable. We can NOT process requests for dental plan cancellation by telephone.
